Any cancer patient who lives in Northeast
Louisiana or who is receiving cancer treatment
in Northeast Louisiana is eligible to complete
an application. The CFL does not help the
families of deceased patients, only the living
patient who is currently receiving treatment.
The CFL provides assistance for cancer related
medication, transportation to/from treatment,
nutritional supplements, medical supplies
and some household bills (examples: mortgage/rent,
utility bills, food, insurance, etc.) The
CFL is not able to pay for doctor bills or
hospital bills as well as bills associated
with phone, cable or utility deposits.
For approved expense requests, the CFL staff
will issue a CFL check for the approved amount
payable to the qualified vendor. Payment
assistance of a check is not made to the
patient.
What Next?
Complete a referral form and
provide written
medical verification of diagnosis
of cancer
and proof of income. The Patient
Services
Committee will evaluate the application.
